The game goes on a 1 to 1 ratio. So 1 game day equals 1 real time day. There are some places that have large "skylights" (that are actually just artificial light sources) that run on a timer to simulate day and night for the characters, so they do have a sense of how long it's been.

At this point it's rather random, we like to try and do one a month though.

For the big quests with a defined goal (for example building Wendy's house in Peter Pan) once the job is done it's done, so only one group of characters can do that quest. The quests show up on the character's Bookmarks as vague suggestions (building Wendy's house was Restore that which is lost) that the characters then figure out and do.
